Would I be correct in guessing this is from a Mega Drive game? The channel spec is rather similar (one empty FM channel, FM6 blocked off, with the use of the DPCM/DAC channel) and the use of 50% duty square waves hint to it.

Nope, not a Mega Drive game, but the original song did use a Yamaha FM chip (the YM2608 ) which was nearly the same as the YM2612, but with more stuff (dedicated ADPCM, three PSG channels built-in, etc.)

I think he composed and/or did the sound programming for some Sega games, like the audio driver for Gunstar Heroes on the Game Gear.

Anyway, I think I'm finished This song is from an obscure 1991 3DRPG called King Breeder, which was developed for the Japanese NEC PC-8801 and PC-9801 personal computers by Artec, and the musical score was composed by Hitoshi Sakimoto and Masaharu Iwata (each did their share of songs). This particular song was composed by Sakimoto, and is my personal favourite in the soundtrack.
EDIT: Changed one instrument to prevent square-wave clashing, and uploaded an NSF file of the song.
