Overall, not terribly bad, there are definately a lot of misplaced notes, such as that square 2 at 1E should be A-2, but you have the correct principal. Also, the tempo should be set at speed 5, not 6.

There are a lot of wrong notes. And when you changed keys, I think most of the notes were wrong. In general, you can avoid a lot of those kind of mistakes by noting what [url=http://en.wikipedia.org/wiki/Key_signature]key signature you're in, and sticking with those notes. (Does the Duck Tales theme have a lot of accidentals?)

It doesn't look like most of the Duck Tales themes have any accidentals. Their key signatures all look steady as far as I can tell.

The chorus/bridge are bit chromatic, consider the sequence of three chords C D and E major. (At about 24 seconds in.)

The piece also shifts up a whole tone at the end, but that's neither here nor there.
