Just in case that wasn't clear enough, open a 2A03 instrument, click the DPCM samples tab, then click the Import button to load a WAV.

Also, Rushjet1 made a stand alone tool that might be easier to use or produce better quality samples: http://famitracker.shoodot.net/forum/posts.php?id=1282
